New DIC claim forms!


The Veterans Benefits Administration, Pension and Fiduciary (P&F) Service would like

to make your organization aware that VA Form 21P-534EZ, Application for DIC,

Survivors Pension, and/or Accrued Benefits was updated with a new version date of

July 2022. With the passing of The Sergeant First Class (SFC) Heath Robinson

Honoring our Promise to Address Comprehensive Toxics (PACT) Act, there were

necessary changes that delayed the release until now. Please note that there have

been significant changes to the form content. These updates were based on the

collaboration that took place during the modernization efforts held with the Pension

Management Centers in 2021. An outline of changes made to this form is addressed in

the attached document.

P&F Service would also like to make you aware that VA Form 21P-535, Application for

Dependency and Indemnity Compensation by Parent(s) (including Accrued Benefits and

Death Compensation When Applicable) was updated to include information on PACT

Act re-evaluation. The newly created Section III allows for a claimant to request a re-

evaluation on a prescribed form if any previously denied Parents’ DIC claim may have

been submitted.

These forms were updated on the VA Forms site.

VA Form 21P-534EZ, Application for DIC, Survivors Pension, and/or Accrued Benefits

July 2022 Change Summary

Intent of the Changes: The application has been restructured to improve clarity and flow for

claimants, claims processors, and data extraction capabilities in support of automation.

• Formatting was updated to include Optical Character Recognition (OCR) to increase available

space for claimants to provide answers. OCR provides clear responses for improved claims


• The instructions have been updated for clarity.

o Attorney, claims agent, and fee language has been grouped together and moved closer

to the beginning of the instructions.

o A table has been included that shows how sections are organized.

o Fully developed claim (FDC) instructions have been updated.

• A page has been added that expands on the terms and calculations utilized in claims


• A checklist has been added for the benefit of the claimants. This checklist is designed to assist

claimants in assembling the needed evidence to substantiate their claim. This checklist should

not be considered evidence for their claim.

• Instructions throughout the application have been updated, to provide the claimant with clarity

on evidence (additional forms or other information) that may be required to complete their


• The Parents’ Dependency and Indemnity Compensation (DIC) Section has been removed, and

applicants are encouraged to complete VA Form 21P-535, Application for Dependency and

Indemnity Compensation By Parent(s) (Including Accrued Benefits and Death Compensation

when Applicable), when applying for Parents’ DIC.

• Service branches have been expanded to include Space Force, National Oceanic and

Atmospheric Administration (NOAA), and United States Public Health Service (USPHS).

• The question prompting for the number of marriages for the Veteran and spouse has been

removed from the application. The claimant is prompted to notify VA of all prior marriages with

sufficient space included to report multiple marriages when applicable. VA will accept the

information provided by the claimant as true and complete unless conflicting evidence exists or

is discovered.

• Section VII of the application now includes an option to request DIC eligibility under The

Sergeant First Class (SFC) Heath Robinson Honoring our Promise to Address Comprehensive

Toxics (PACT) Act (PL 117-168), as an election to re-evaluate a previously denied claim. This

is located within the section including 1151 consideration.

• The income and assets section has been restructured to capture net worth information and

may not require the submission of VA Form 21P-0969, Income and Asset Statement in

Support of Claim for Pension or Parents' Dependency and Indemnity Compensation (DIC)

(Attachment to VA Forms 21P-527, 21P-527EZ, 21P-534, and 21P-534EZ). The application

was updated to include up to four income sources.

• Medical expenses have been updated and split to better capture continuing care expenses (in-

home care, assisted living facility, etc.) and other medical expenses.

• Signature blocks have been expanded to include an alternate signature block.

• The facility care and in-home care worksheets have been redesigned to act as expense
